Separate Retrieval Failure From Evidence-Use Failure

A wrong answer in a retrieval workflow does not identify which stage failed. The relevant passage may never have been retrieved, may have been discarded, or may have been supplied and then used incorrectly.
Quick answer: Compare the source material, retrieval result, selected context and final answer as separate artifacts. Check whether the needed evidence existed, was accessible and reached generation. Use controlled context substitution to investigate evidence use, while preserving the original request and execution conditions.
Use a revision-specific question
For an illustrative policy assistant, ask about a rule in a named document revision. The controlled source fixture contains the answer in a specific passage.
If retrieval returns a different revision, inspect filters and indexing context. If retrieval returns the right passage but the context builder removes it, investigate selection or truncation. If the passage reaches generation and the answer contradicts it, investigate how that evidence was used.
Preserve the intermediate artifacts

These distinctions prevent a generic “hallucination” label from becoming the whole diagnosis. They do not expose a model's hidden internal process.
Test a narrower hypothesis
Supply the known relevant passage directly in a controlled rerun. If the answer improves, that is evidence worth investigating about the retrieval path. It is not proof that every earlier failure had the same cause.
Keep a case where the requested fact is absent. An assistant that always produces a confident answer can appear improved on answerable questions while becoming less trustworthy on missing evidence.
Name the next engineering action
A fix might concern revision filtering, context assembly, citation mapping or response behavior. Assign the task to the stage supported by the evidence.
If source access failed, keep that operational result separate from an incorrect answer produced from successfully retrieved material.
