When an Agent Error Message Is Insufficient Evidence

“The tool failed” may be accurate, but it rarely identifies the next engineering action. The same visible message can represent a rejected request, an expired deadline or an operation whose outcome is still unknown.

Quick answer: Preserve the operation name, attempt identifier, relevant arguments, response classification and timing around an error. Check the provider's documented semantics. Distinguish confirmed rejection from uncertain completion before retrying a state-changing operation.

Ask what the message actually establishes

Consider an illustrative scheduling tool that times out after a request is submitted. The client may know that it did not receive a response. It may not know whether the appointment was created.

Changing the final answer from “failed” to “try again” does not resolve that uncertainty. Inspect a documented operation-status mechanism or other appropriate state evidence before another attempt.

Build an investigation packet

ObservationWhy it matters
Operation and attemptIdentifies which action encountered the problem
Relevant parametersDistinguishes invalid input from a valid request
Provider status and bodyPreserves documented error information
Deadline and elapsed timeHelps locate where the attempt ended
Retry historyShows whether another request may already exist
Outcome evidenceDistinguishes unknown from confirmed failure

Exclude credentials and unnecessary personal content. Store sensitive evidence only in an appropriately restricted location.

Preserve uncertainty in the response

A useful user explanation may say that the operation's completion could not be confirmed. It should not assert that nothing changed unless the evidence supports that conclusion.

For HTTP requests, RFC 9110's idempotency definition concerns the intended effect of repeated identical requests. A specific API's replay and deduplication behavior still requires its own contract.

Turn the finding into a test

Use a controlled fixture that withholds an acknowledgment while retaining an inspectable operation state. Check whether the assistant reports the uncertainty and follows the intended recovery path.

This tests recovery behavior without pretending that a generic timeout label proves a root cause.
